    No, you take a whole week and find, in three separate days, the amount of weight you can lift for each rep range and for each exercise individually and preferably in the order you're planning to perform them. Have you read the HST articles?

You take 9-12 days off anfd then start a load progression untill at workout six you hit your 15 RM, which is the maximum weight you can lift for 15 reps.     Yeah but since you're not really fighting inertia (spelling?) it's not the same thing at all. The acceleration part is what kills you when sprinting, and since in the threadmill you're not moving it doesn't compare. And, at least in my case, I've never seen one that would go as fast as I can...

    I only do them once I'm into five's, but my five's nowadays last forever. The only problem with doing it during fifteens is that it will somewhat affect decondicioning, especially if to short sprints are used at near maximal intensity.

    Sprinting outdoors would be your best bet, but you can also do it in a stationary bike especially in one of those used in spinning classes, the regular ones at the cardio section in gyms are not as suited to that.     It is weird but I once screwed my back doing Smith Squats. It was the only back injury I ever had and regular Squats never gave me any trouble, other than normal lower back soreness.
